中文摘要 忽略症為無法注意及察覺到腦傷對側空間之刺激的症狀,已被認為是影響中風患者日常生活功能與導致復健成效不佳的預測因子之一。目前研究已證明音樂能增進健康受試者或是臨床之病患認知功能,包括注意力。音樂的特色為能使聆聽者產生愉悅感受,而根據正向情緒之擴建理論的主張,正向情緒能擴展個體之注意力的範圍,因此忽略症患者可能可藉由情緒的改變來調節其注意力,進而改善忽略症狀。然而,針對此議題的相關研究十分有限,本研究之目的為探討不同類型之音樂刺激於提升忽略症患者腦傷對側刺激之察覺和注意力的效果;並進一步探討由音樂刺激引發之愉悅性對提升忽略症個案注意力之加乘效果。共有19位中風且具忽略症之患者參與本研究。主要以紙筆測驗、視覺搜尋作業和偵測作業檢驗患者在四種不同情境(包括白噪音、愉悅音樂、不愉悅音樂和古典樂)下對其忽略症狀的影響效果。本研究以視覺類比刻度尺、皮膚表面電位與心跳速率評估受試者由音樂所引發之不同情緒的反應。此外,在視覺搜尋作業中同時使用眼球追蹤系統直接測量患者之注意力分佈情況。研究結果顯示,自選之愉悅音樂能引發患者的正向情緒且調節警醒程度,不論在多數紙筆測驗的分數或是量化的眼球動作數據皆發現顯著向左注意的增進效果,減緩其忽略症狀。本研究的結果證實愉悅音樂能改善忽略症之注意力表現,並可進一步協助臨床治療師以愉悅音樂作為治療媒介,對忽略症患者發展有效的個別化治療策略。
英文摘要 Neglect can generally be defined as failure to attend to the contralesional side of space. This syndrome is a well-recognized predictor of poor functional outcome following unilateral stroke. Recent studies have suggested that music can enhance cognitive functions in healthy subjects and clinical conditions, and also can produce pleasure or positive emotion. Furthermore, the broaden-and-building theory assumes that positive emotion can broaden the scope of attention. According to this hypothesis the attentional functions in patients with neglect may be modulated by the emotion. However, research on emotion induced by music in modulating neglect is very limited. The main aims of the present study are to examine what kind of music can increase awareness of contralateral stimuli in neglect and further to investigate whether the pleasure induced by music can produce a greater effect. A total of 19 stroke patients with neglect participated in this study. The paper and pencil tests, visual search task, and detection task were be used to examine the modulation effects of neglect in four different conditions (white noise, pleasant music, unpleasant music, and classical music condition). The pleasure induced by different music was assessed by the visual analog scale, galvanic skin response and heart rate. In addition, the eye tracking system was used in visual search task to measure visual attention distribution directly. Results showed that pleasant music generated positive emotion, modulated arousal, which further improved attentional performance in standardized tests and quantitative eye movement data. These findings not only demonstrate that positive emotion, induced by pleasant music, can ameliorate neglect by increasing attentional resources, but also facilitate therapists to develop more efficient individualized treatment strategy for neglect patients by using pleasant music.
