中文摘要 背景:儘管中年期婦女之更年期研究已建立豐富的知識基礎,但仍少有研究同時探討更年期症狀、憂鬱症狀、睡眠品質及生活品質之關係,且男性方面之相關研究亦為罕見,甚至兩性在這些變項間之差異分析在國內更是闕如。
目的:分析中年人之更年期症狀、憂鬱症狀、睡眠品質與生活品質之性別差異。
方法:本研究採橫斷性調查研究法,方便取樣年齡介於40-65歲社區男女共725位作為研究對象,使用之問卷包括:基本資料、更年期症狀量表、流行病學研究中心憂鬱量表、匹茲堡睡眠品質量表和台灣簡明版世界衛生組織生活品質問卷。
結果:研究結果顯示:1.兩性的人口統計學分佈不盡相同,包括:年齡、BMI、婚姻狀況、工作狀況、女性更年期知識來源、更年期症狀處理方式、運動習慣、飲食習慣、菸酒使用狀況、自覺生活壓力分數、壓力源及紓壓方式;2.整體更年期症狀、憂鬱症狀、睡眠品質和生活品質之得分方面均無性別差異,但研究進一步發現:經常性哭泣、感覺眩暈或無力、頭痛、熱潮紅和性興趣喪失之更年期症狀,以及睡眠遲滯期,在兩性之間則均具有顯著差異;3.中年期男女、中年期男性和中年期女性三組的生活品質影響因素及預測因子有所不同。中年期男女生活品質之預測因子包括:更年期症狀、憂鬱症狀、睡眠品質、全家平均月收入、自覺生活壓力分數、每次運動時間和BMI值,共可解釋生活品質50.8%的變異量;中年期男性生活品質之預測因子包括:更年期症狀、憂鬱症狀、睡眠品質和教育程度,共可解釋生活品質51.6%的變異量;中年期女性生活品質之預測因子包括:更年期症狀、憂鬱症狀、睡眠品質、全家平均月收入、自覺生活壓力分數、飲食習慣和停經階段,共可解釋生活品質52.7%的變異量。
結論:研究結果呈現兩性於更年期症狀、憂鬱症狀、睡眠品質和生活品質方面之異同處,除了有助於健康照護專業人員發展合適的照護策略外,同時也證明性別分析的必要性及提供不同照護需求之重要性。
英文摘要 This study analyzed the gender differences in climacteric symptoms, depressive symptoms, sleep quality and quality of life among middle-aged dwellers. We used a cross-sectional method. Participants were convenience sampled from a residential community in Taiwan. A total of 725 middle-aged dwellers between 40 and 65 years of age participated in the study. Five measures were used: a demographics and health habits questionnaire, the Climacteric Symptom Scale, the Center for Epidemiological Studies-Depression Scale, the Pittsburgh Sleep Quality Index and the WHOQOL-BREF Taiwan version. The results revealed that: (1) Significant demographic differences between females and males. (2) No significant differences between females and males in climacteric symptom, depressive symptom, sleep quality and quality of life. Our study provided further findings that females reported significantly more severe climacteric symptom of crying spells, feeling dizzy or faint, headaches, hot flashes and loss of interest in sex than did the males. The females also reported significantly longer sleep latency than did the males. (3) There were some differences in factors and predictors of quality of life among three groups. Results may assist healthcare professionals in developing appropriate care strategies, and it evidenced the importance of gender analysis and provide diverse care between females and males.
