中文摘要 近年來台灣陸續進行許多與年長者功能性體適能之相關研究,但目前尚缺乏完整的本土常模可作為參考,也較少研究針對年長者功能性體適能與現階段運動行為階段以及自覺健康狀況之關係的相關探討。目的:(1)以台南市年長者為對象,建立年長者功能性體適能常模,並比較年長者男女性在老化趨勢的差異性,(2)比較本研究常模與國內外相關研究之差異(3)探討年長者功能性體適能與現階段運動行為以及自覺健康狀況之關係。方法:以1,388位台南市60歲以上,居住在台南市社區為主的年長者為研究對象,平均年齡為72.2歲,以分層抽樣方式,實施一次性的基本資料問卷後進行年長者功能性體適能測驗。研究結果以描述性統計、卡方百分比同質性檢定、百分等級、單因子變異數分析、獨立樣本二因子變異數分析與逐步多元迴歸分析所得資料。結果:(1)年長者功能性體適能除了BMI以外,其餘體適能結果從75-79歲開始顯著下降,80歲之後體適能情況更與60-79歲等四期年齡層有顯著差異(p<.05)。(2)本研究各年齡常模之平均值在上下肢肌力、心肺耐力、女性握力及女性上肢柔軟度表現較好;美國在敏捷/動態平衡及男性上肢柔軟度較佳;台北大安社區則是在女性下肢柔軟度較好。(3)處於不同運動階段的年長者,在上下肢肌力、慣用手前臂肌力、上下肢柔軟度、心肺耐力以及敏捷性/動態平衡有顯著的差異(p<.05)。(4)自覺健康較好之研究對象其功能性體適能項目中的上下肢肌力、慣用手前臂肌力、心肺耐力與敏捷/動態平衡也顯著高於自覺健康狀況較差者(p<.05)(5)下肢肌力、自覺健康狀況與教育程度為預測現階段運動行為的主要因子(6)年長者自述是否有慢性病、現階段運動行為、上肢肌力及性別為自覺健康狀況的主要預測因子。結論:各項測驗常模除了BMI值之外,其他項目之功能性體適能皆隨年齡上升而下降。男性在肌力項目較女性佳,女性則是柔軟度優於男性。此外,年長者處於不同運動行為階段與自覺健康狀況會受到部分功能性體適能之影響,其中以三十秒起站測驗所代表的下肢肌力及三十秒手臂屈舉的上肢肌力的功能性影響最為直接。本研究發展之年長者功能性體適能常模除可應用在相關老年研究議題上,,並幫助相關學者更深入探討老化的趨勢與相關議題的參考。
英文摘要 Background: Recently, many related researches about senior functional fitness had been successively developed, but still lack a complete reference for Taiwanese seniors. In addition, the related studies about the relationship between senior functional fitness and stage of exercise behavior and perceived health status were still absent. Objective: The purpose of this study were (1) to establish the normative scores of senior functional fitness and compare the differences between gender in aging tendency for community-dwelling older adults in Tainan city, (2) to compare this study with US norms and two studies in Taiwan, (3) to verify the relationship between senior functional fitness and exercise behavior and perceived health status. Method: A total of 1,388 participants (72.2±7.2 years) were recruited using stratified random sampling from the 60-95-year-old community- dwelling older adults. After completing the basic information questionnaire, performed the assessment of senior functional fitness. Data were analyzed by descriptive statistics, chi-square test of homogeneity proportions, percentile, one-way ANOVA, two-way ANOVA and stepwise multiple regression. Results: (1) In the senior functional fitness test, except BMI, all the collected data decreased significantly from age 75-79 years. At the aged 80s, the decline tendency were much more significantly than the former four disparity ages (p<.05). (2) Compare to the subjects in Tainan demonstrated better upper and lower body muscle strength, cardio-respiratory endurance in males, and females had greater hand grip strength and upper body flexibility. However, the norm of America was superior in agility/dynamic balance and male upper body flexibility. In Daan community, female elders had a better score in lower body flexibility; there was a better score in male hand grip in Tianliao distinction. (3) There were significant differences between different stages of exercise in upper and lower body muscle strength, hand grip, upper and lower body flexibility, cardio-respiratory endurance and agility/dynamic balance. (4) The person who perceived health status was better than the others had better functional fitness in upper and lower body muscle strength, hand grip, cardio-respiratory endurance and agility/dynamic balance. (5) Lower body muscle strength, perceived health status and level of education were the mainly predicted factors of stage of exercise behavior. (6) Seniors self-express whether suffered from chronic diseases, stage of exercise behavior, upper body muscle strength and gender were the predominantly predicted factors of perceived health status. Conclusions: Except BMI, all the norms of senior functional fitness significantly decrease with increased age. While the males was better in muscle fitness, and females was superior in flexibility. Senior functional fitness performance influenced by different stage of exercise behavior and perceived health status. The lower body muscle strength (represented by 30s chair stand test) and upper body muscle strength (represented by 30s arm curl test) were the most direct influenced factors. The development of senior functional fitness normative scores might be applied in related aging research areas. It could not only explore the tendency of senescence but also help scholars discuss aging-related issues more profoundly.
