中文摘要 文獻指出,輕度認知障礙者(mild cognitive impairment)或失智症者皆可能有視知覺與視覺注意力問題,但大多研究聚焦於特定面向,較缺乏一廣泛性的視知覺能力評估。此外,亦不清楚這些能力的缺損對於日常生活功能的影響性。有鑑於此,本研究目的為(1)比較輕度認知障礙者、失智症者與健康者之視知覺與視覺注意力缺損面向(2)檢驗視知覺能力、視覺注意力與日常生活功能之相關性。
本研究採描述性研究與探索性研究並行,招募共102位受試者,在排除不合適者後,得健康者25位、輕度認知障礙者34位與失智者12人,納入後續分析。使用Kruskal-Wallis test以及線性迴歸分析三組之視知覺、視覺注意力、以及日常生活功能之差異。使用斯皮爾曼相關性分析以及階層迴歸模型分析人口學變項、視知覺、視覺注意力與日常生活功能的相關性。
結果顯示,在控制年齡與教育程度後,失智症組於基本視覺處理能力、空間知覺、視覺注意力、視覺認知能力較輕度認知障礙組、健康組差,但於物體知覺方面則無顯著差異。輕度認知障礙組於空間知覺、視覺注意力、視覺認知能力較健康組差。失智症組之物體知覺與其他二組無顯著差異,推測可能受到其低教育程度以及高齡的影響較大。視知覺、視覺注意力與日常生活存在低度到中度的相關,但在考量人口學資料後,僅基本視覺處理能力、視覺注意力、視覺認知功能對日常生活功能有顯著性的影響。物體知覺與空間知覺未有顯著性的影響,推測可能是其能力較為基本,故未能呈現顯著影響力。
未來研究可再招募高教育程度且年紀較低的失智症者,或低教育程度且年紀較高的輕度認知障礙者、健康者,以驗證本研究之結果。
英文摘要 Literature has shown people with mild cognitive impairment (MCI) or dementia have visual perception (VP) and visual attention (VA) deficits. However, most studies investigated certain VP/VA domains and did not explore the spectrum of VP/VA associated with MCI or dementia. There is also limited understanding about the impact of VP/VA deficits on daily function. This study is to (1) compare the pattern of VP/VA deficits in older persons with MCI, dementia and normal cognition (NC), and (2) examine the relationship between VP/VA and daily function. We adopted a descriptive and exploratory design. 102 participants were recruited and 25 older adults with NC, 34 with MCI, 12 with dementia remained for data analysis. Kruskal-Wallis test and linear regression were used to determine whether difference existed among three groups in VP, VA, and daily function. Spearman’s correlation and hierarchy regression were used for exploring the relationship among demographics data, VP, VA, and daily function. The results showed the dementia group performed worse on basic visual process ability, spatial perception (SP), VA, and visual cognition (VC) than MCI and NC group, except object perception (OP). The MCI group performed worse on SP, VA, and VC than NC group. The reason that dementia group have no difference on OP with MCI or NC group may due to the effect of higher age and lower education. VP/VA had low to moderate correlation with daily function. After adjusting for demographics, only basic visual process ability, VA and VC were correlated with daily function. The reason OP/SP have no significant influence on daily function may due to these abilities are more basic. Future study could recruit participants with dementia with higher education level, lower age or participants with MCI/NC with lower education level, higher age to confirm the study’s results.
