英文摘要 ABSTRACT
Background: Diabetic foot ulcer (DFU) is one of the serious complications for patients with diabetes mellitus (DM). The quality of life (QoL) of patients with DFU is worse than patient with DM without foot ulcer and the general population. Women with DFU also had poorer QoL than men with DFU. Foot ulcer makes women in Indonesia loss opportunity to participate religious activities due to unable to use beautiful footwear. The body image may be one factor make women had poor QoL than men with DFU in Indonesia.
Purpose: The purpose of this study is to examine body image as a mediator between gender and QoL among patients with DFU in Indonesia.
Method: Cross-sectional and descriptive correlation design with convenience sampling was used at Surgical Outpatient Department as well as wound care clinic in Bali of Indonesia. A structured questionnaire including the Diabetic Foot Ulcer Scale-Short Form (DFS-SF), Body Investment Scale (BIS) was administrated. Descriptive statistics were used to identify the demographic and clinical characteristic. Independent t-test was used for specific aims such as understand QoL and body image among women and men with DFU. PROCESS macro model 4 was used to examine the effect of body image as a mediator between gender and QoL.
Results: There were 201 participants of which 103 (51.2%) women and 98 (48.8%) men in this study. This study found that there were gender differences in their QoL and body image domains (p<.05) in which men had higher body image score and QoL than women. Body image has completely mediated the effect of the relationship between gender and QoL significantly explained (B=6.68; 95%CI=3.14 to 10.52).
Conclusion & Implication: Women with DFU had poor body image and QoL than men. The relationship between body image and QoL for women is stronger than men. And this study validated that body image as a mediator between gender and QoL. Health care providers need to approach holistically to solve the body image problem among women with DFU during wound care and modify the footwear.
