中文摘要 研究背景與目的:
『輕度認知障礙』為正常老化到失智症的過渡階段,由於認知功能範疇涵蓋語言表達、注意力、視覺空間和執行功能,這些都是維持個體獨立日常生活基本要素,間接影響生活品質,因此有輕度認知障礙的長者之生活品質不容小覷。故本研究將探討社區輕度認知障礙長者之生活品質並探知生活品質在不同認知退化範疇上的差異;以及探討於控制個人屬性後,此族群長者的認知功能對生活品質的影響。
研究方法:
本研究為前瞻性的研究採橫斷式研究設計,資料來自大台南市之社區民眾。以方便取樣方法,配合社區舉辦健康篩檢活動,針對60歲及以上自願民眾,以聖路易大學心智狀態測驗(SLUMS)進行認知功能初篩,世界衛生組織生活品質問卷-台灣簡明版(WHOQOL-BREF TW)收集參與個案資料。以描述性統計法分析個案基本屬性資料、認知功能狀況和生活品質狀況;再以推論性t test與ANOVA等統計方法分析生活品質在不同認知功能範疇上的差異;最後,控制基本屬性等干擾變項後,以複迴歸分析認知功能對生活品質的影響。
研究結果:
本研究發現輕度認知障礙族群在生活品質的心理範疇偏低,而生活品質對認知功能個範疇含執行功能、記憶力、計算力、數字序列、回覆能力、命名能力、
定向力、圖像識別退化未造成影響。此外,研究結果發現整體認知功能狀況無法預測此族群的整體生活品質狀況。
預期貢獻:
本研究結果有助於提供未來我國社區基層醫療單位與民間團體規劃認知功能障礙長者照護之參考,期落實早期查覺社區長者認知功能障礙與早期介入預防與照顧策略,研擬維持輕度認知障礙長者之生活品質方案,進而延緩其真正進入失智狀況並減少醫療與社會成本的支出。
英文摘要 Background
Mild cognitive impairment is an intermediate stage between normal aging and dementia. Cognitive function includes language expression, attention, visual-spatial ability and executive functions which are not only basic elements of maintaining an individual's independent daily life but also indirectly affect quality of life. The purpose of this study was to explore quality of life among older adults with mild cognitive impairment in community and to examine the influence of various cognitive impairment domains on quality of life among this group of people. Furthermore, whether cognitive function predict quality of life after controlling the confounding variables was analyzed.
Method
This study was a cross-sectional design and prospective study, data were collected from community-dwelling older adults in Tainan. Convenience sampling was used, and subjects were volunteer people who were 60 years old and older. Saint Louis University Mental Status Examination tool was used to assess cognitive status of the subjects and qulaity of life was assessed by World Health Organization Quality Of Life -BREF Taiwan version. Data were analyzed by description, t-test statistics, and regression method.
Result:
Results showed that MCI subjects have lower score on the mental domain of quality of life. However, all domains of cognitive function (memory, caculation,
recall, name, oritention, digit span and figure identification, executive functions) have no influence on quality of life. Besides, overall cognitive function cannot predict subjects’ overall quality of life.
Conclusion
Findings of this study will provide references for community healthcare providers and organizations when planning for mental care for older adults. Through prevention and care in early stage, the quality of life of elderly with mild cognitive impairment could be maintained. It is hoped to delay the onset of dementia and reduce the medical and social costs.
