中文摘要 背景和目的:踝關節扭傷是一種常見的運動傷害。多數運動員在外側踝關節扭傷之後常常惡化為慢性踝關節不穩定。此類患者發生重覆性扭傷多數都是在執行有著地任務的運動動作中,然而踝關節在不同的動作任務中所需要的穩定度和活動度也有所不同。因此本實驗的主要目的是針對慢性踝關節不穩定之患者,於不同運動動作中執行著地任務時的踝關節穩定性之評估和量化,以及在執行動態平衡測試的表現。方法:15位慢性踝關節不穩定的患者及15位健康且年齡相符的運動員參與本實驗。所有的受測者須接受下肢肌力、踝關節活動度和動態平衡測試以及完整執行四種運動動作。其中動態平衡測試為測試腳(踝關節不穩側)於單腳站姿下,其非測試腳作八個不同方向的最長距離伸取。而四種運動動作包含:跑步、急停跳躍、 切入後90度急轉、側移。我們分別使用紅外線高速攝影機、力板和表面肌電圖系統來收取實驗相關的資料。我們使用踝關節力矩和角度的關係來計算踝關節的動態關節韌度,以及分析兩組拮抗肌(前脛肌與腓長肌;前脛肌與腓腸肌)的共同收縮指數在執行不同運動動作時著地前後的兩組差異。此外,我們一併分析各動作的生物力學參數包含:踝關節角度、前足相對於後足角度、後足相對於小腿角度、地板反作用力和前脛肌、腓長肌與腓腸肌的激化程度等。結果: 在執行急停跳躍和側移動作的著地前期,慢性踝關節不穩定的患者的前脛肌與腓長肌之共同收縮指數比健康的運動員較低;而在執行跑步的著地前期,慢性踝關節不穩定的患者有明顯的踝關節的內翻角度過大;於著地後期,其動態踝關節韌度比健康的運動員較低。此外,慢性踝關節不穩定的患者在蹠翻肌力比較差而且在動態平衡測試時的伸取距離也比較短。結論:在執行運動動作時,慢性踝關節不穩定的患者會在著地前其控制踝關節內外翻肌的共同收縮程度下降而著地後其動態關節韌度下降皆是降低踝關節穩定度的可能情況,以及著地前踝關節呈現過度內翻角度,這些現象有可能會提高腳踝再次扭傷的危險性。此外,實驗結果也顯示慢性踝關節不穩定的患者有蹠屈肌力缺失與動態姿勢控制上的問題。根據這些結果,針對慢性踝關節不穩定患者之神經肌肉控制再訓練的設計,治療師可以從動態平衡能力與動態關節穩定性兩大方面去改善。
英文摘要 Background and purpose: Ankle sprain is a common sport injury observed in the athletes. Majority of athletes with a lateral ankle sprain would develop into the chronic ankle instability (CAI). Recurrent ankle sprain usually occurred in the individuals with CAI at landing during sports activity. However, different sporting movement requires different demand for maintaining the stability and mobility of the ankle joint. One purpose of this study was to evaluate and quantify the stability of the ankle joint in landing task of sport movements in the individuals with CAI. The other purpose was to investigate the performance of the subjects with CAI during the dynamic balance test. Method: Fifteen subjects with CAI and 15 uninjured and age matched athletes were recruited in this study. All subjects were asked to perform four sport maneuvers including running, stop-jump, cutting and lateral moving. In addition, the leg-reaching tasks in eight directions in the dynamic balance test were also executed. Eight infrared video cameras, two Kistler force plates and a 16-channel surface EMG system were used to record the experimental data. The dynamic ankle joint stiffness, the co-contraction index of tibialis anterior (TA) / peroneus longus (PL) and TA/ gastrocnemius lateralis (GL), ankle joint angle, rearfoot to forefoot angle, rearfoot to shank angle, ground reaction force (GRF), and the root-mean-square (RMS) of TA, PL and GL were measured during sport maneuvers. The reach distance, COP displacement, the muscle activities during the dynamic balance test were processed. Results: The subject with ankle instability had less muscle co-contraction index of TA and PL during the landing task of stop-jump and lateral moving in the phase of pre-landing. During running, the CAI group had greater ankle inversion angle in the pre-landing phase and less dynamic ankle joint stiffness in the post-landing phase compared with the control group. The subject with chronic ankle instability had weak ankle plantarflexor and less reach distance in the most directions of the SEBT. Conclusions: The decreased muscle co-contraction in the ankle joint at pre-landing phase and the decreased dynamic joint stiffness at post-landing phase may jeopardize the stability of the ankle joint. Furthermore, landing with excessive ankle inversion angle may predispose the individual to recurrence of ankle sprain during sport maneuvers. The poor ankle plantarflexor and dynamic postural control was also found in the subject with CAI. Therefore, the therapist may design a neuromuscular retraining that lay emphasis on the improvement of dynamic postural control and dynamic joint stability of the subjects with CAI.
