中文摘要 研究背景及目的:全世界罹患糖尿病的族群中約有90%屬於第二型糖尿病,而第二型糖尿病的病程發展到後來,容易因周圍神經病變造成感覺遲鈍,肢段受傷不自知而引起傷口感染,導致細菌進入血液而產生內毒素血症;嚴重的內毒素血症會造成血液灌流不足的低血壓及心肺功能衰竭,而內毒素引起的全身性發炎反應,會產生腫瘤壞死因子(TNF-α)等發炎前驅物質,進一步引起肺部損傷和多重器官衰竭。運動是第二型糖尿病患的主要治療方式之一,而胰島素阻抗為第二型糖尿病發展之初最主要的症狀之一;透過運動訓練或熱刺激能在大鼠多重器官內,誘發保護性熱休克蛋白72(HSP72),保護受到破壞的蛋白質以維持其正常的功能。
英文摘要 Background and Purpose: Diabetic patients are susceptible to E. coli lipopolysaccharide (LPS) infection, resulting in endotoxemia with cardiopulmonary failure and high mortality rate. We have also demonstrated that heat shock protein-72 can be detected in various organs from normal and type I diabetic rats with exercise training. The purpose of the current study was to characterize effects of exercise preconditioning on endotoxemia-induced cardiopulmonary dysfunction in the rat model of type II diabetes. Methods: Aged 6 weeks male Wistar rat fed 60% fructose-rich chow for 6-8 weeks. When loss of plasma glucose lowering response to tolbutamide were considered as type II diabetes with insulin resistance and randomly assigned to sedentary or exercise groups. The trained rats run gradually on a treadmill 5 days/week, 30-60 min/day with intensity 20-30m/min for 4 weeks. After 24 hrs from the last training session, we compared the temporal profiles of mean arterial pressure, heart rate, cardiac output, and stroke volume of urethane-anesthetized rats after intravenous injection of LPS. At the 4th hour after LPS injection, we determined arterial blood gas, tumor necrosis factor-alpha level in serum and lavage and lung edema. In addition, the HSP72 expression in multiple organs were determined in different groups. Results: After exercise training, we found that HSP72 expression in multiple organs were significantly greater in rats with exercise training than sedentary rats. The survival time was significantly longer in exercised-diabetic rat after administration of LPS. During endotoxemia, prior exercise training could significantly attenuate tumor necrosis factor-alpha level in serum and lavage, diminish the degree of lung edema and prolong the survival time of rats. Conclusions: The current study showed that exercise pretraining prolonged survival time and increased survival rate of fructose-fed diabetic rats after administration of LPS. Clinical Relevance: We provided important information for disclosing the effects of exercise for type II diabetic patients.
