中文摘要 超越老化為老人轉變對現實生活的看法,是一種正向及成功的老化觀感。但此階段發展並非人人可及,在老化過程中,老人可能因身體功能退化、憂鬱、缺乏社會支持系統或罹患疾病的衝擊等因素,而阻礙其超越老化的發展。故本研究旨在了解機構老人自我超越老化觀感的情形,並探討老人在身、心、社會、靈性各層面的狀況與超越老化觀感之相關性,即基本屬性、日常生活活動功能、憂鬱狀況、社會支持度、靈性層面及生活滿意度與超越老化之關係,並進而探知影響機構老人超越老化觀感的重要因素。
本研究為一描述相關性研究,以叢聚取樣、結構性問卷調查150位台南縣市五所長期照護機構內65歲以上且認知功能正常的老人。研究工具包括老人基本屬性調查表、超越老化量表、巴氏日常生活功能量表、老年憂鬱量表簡明版、社會支持度量表、生命意義量表及生活滿意度量表。所收集到的資料以描述性統計,含百分比、次數分配、平均值、標準差,及推論性統計,含獨立樣本t檢定、單因子變異數分析、皮爾森積差相關分析及複迴歸分析進行分析。
研究結果顯示台灣老人之超越老化觀感普遍偏低,且老人的超越老化觀感與其性別、教育程度、婚姻狀況、社會經濟狀態、宗教信仰、入住機構前居住狀態及兩年內是否有經歷特殊事件有顯著差異性 ( p< .05 )。此外,超越老化觀感又與社會支持度、靈性層面及生活滿意度呈正相關 ( p< .01 ),然與憂鬱狀況呈負相關 ( p< .01 ),顯示當機購老人社會支持度越高、靈性發展越成熟及生活滿意度越好,其超越老化觀感越佳,反之,若老人憂鬱情形越嚴重,則超越老化觀感越差。研究結果另發現機構老人的靈性、生活滿意度、教育程度、罹患慢性病數、性別對老人自身超越老化觀感具顯著預測力,此五項變項解釋超越老化觀感總變異量為52.8%,顯示此五變項為影響老人超越老化觀感的重要因素。本研究之超越老化觀點及其影響的相關因素探討,不僅協助專業照護者對機構老人在生理、心理、社會、靈性與超越老化觀感之間關係有更全面的認知,更有助臨床照護者進行相關活動設計或護理措施,進而協助機構老人對老化及機構入住的適應,以促進其晚年生活品質。
英文摘要 Gerotranscendence is a shift of meta-perspective in later life which enable elders to experience successful aging and late-life growth. However, physical function deteriorations, depression, poor social network, physical illness in later life can hinder elders to pursuit gerotranscendence status during the aging process. The purpose of this study was to investigate the institutional elder's self perception toward gerotranscendence and to explore the relationship among elder's demographic characters, activity of daily living function, depressive symptoms, social support, spirituality, life satisfaction and gerotranscendence, and to identify the important factors of gerotranscendence.
This was a descriptive-correlational research using cluster sampling and structural questionnaire to collect 150 elderly residents who were older than 65 and cognitively intact from five long-term care facilities in southern Taiwan. Instruments included Basic Demographic Sheet, Gerotranscendence Scale, Bathel Scale, Short-Form Geriatric Depression Scale(SF-GDS), Social Support Scale, Life Meaning scale, and Life Satisfaction Scale. Data were analysed by descriptive statistics including percentage, frequency, mean, standard deviation; and deductive statistics including independent-samples t test, one-way ANOVA, Pearson correlation, and multiple regression.
Findings showed that elders in Taiwan had lower level of gerotranscendence. Significant differences in gerotranscendence in respect to sex, education, marital status, social economic situation, religious belief, previous living condition, and particular events within two years experienced were found (p< .05). Moreover, significant positive relationships between gerotranscendence and social support, spirituality, and life satisfaction; and significant negative relationship between gerotranscendence and depressive symptoms were found (p< .01), indicating that institutional elders’ with higher social support, great spirituality, and better life satisfaction, experience better perception of gerotranscendence. On the contrary, institutional elders with severe depression, experience lower level of gerotranscendence. Results also showed that spirituality, life satisfaction, educational level, numbers of chronic disease, and sex were the significant correlates of gerotranscendence, these five variables totally accounted for 52.8% of the variance, indicating that these five variables are important influencing factors for gerotranscendence in elders. Findings of this study not only provide health professionals the knowledge basis on the relationships between the elders’ physical, mental, social, spiritual dimension and gerotranscendence, but also assist clinical caregivers to plan for activities and nursing interventions. It is believed that such findings will be helpful for institutional elders to adapt aging process and relocation, so that their quality of life cam be improved.
