中文摘要   近年來針灸常被廣泛的應用在臨床的疾病治療,特別是針對止痛與調節內臟的功能。到目前為止,有許多的文獻指出針灸是透過神經系統而達到治療的效果。雖然針灸的作用機制在電生理與神經病理學上已經被建立了,但是,單一個穴位點與其對應臟器之間的神經連結,所知仍然有限。在傳統的中國針灸醫學上,公孫穴主要被運用在治療一些骨盆腔與婦產科相關的疾病,像是難產與經痛。但是,公孫穴與子宮之間的神經連結徑路,是否單獨透過脊髓的神經迴路,或是需要經過更高階的神經支配,目前為止仍有爭議。因此,本實驗的目的主要去釐清公孫穴與子宮之間的神經連結。
  首先,我們在雌性大白鼠的公孫穴上給予電針灸的刺激,藉此誘發神經細胞活化而產生c-fos蛋白,然後利用免疫化學組織法去偵測c-fos免疫反應的細胞在不同層級的神經系統中的表現。另外,我們將豬假性狂犬性病毒 (PrV)這種病毒性的神經追蹤劑注入子宮內,藉此觀察與支配子宮相關的神經組成與中樞調控的神經核。最後,進一步的利用c-fos與PrV的雙重免疫化學組織標定,以此來建立公孫穴與子宮之間神經的連結徑路。
  本實驗的結果顯示,在脊髓的區域具有c-fos免疫反應的細胞主要分佈在L2到L5脊髓節段的lamina I、II、IV、V 、X,而被PrV感染的細胞主要出現在T11到S2的脊髓背角以及lamina X和L6到S2與T11到L3的脊髓外側角。在延腦的部份,具有c-fos免疫反應的細胞主要出現在gracile nucleus、 gigantocellular reticular nucleus、parvocellular reticular nucleus和lateral reticular nucleus。另外,被PrV感染的細胞主要出現在nucleus tractus solitarius、 dorsal motor vagal nucleus、 gigantocellular reticular nucleus、 parvocellular reticular nucleus以及lateral reticular nucleus。在間腦的部份,具有c-fos免疫反應的細胞會出現在thalamic midline nucleus、suprachiasmatic nucleus of hypothalamus、paraventricular nucleus、anterior perforating substance、 habenular nucleus和amygdala,而被PrV感染的細胞會出現在thalamic midline nucleus、habenular nucleus以及paraventricular nucleus。最後,在延腦與間腦同時具有c-fos免疫反應與被PrV感染的細胞只會出現在gigantocellular reticular nucleus、 parvocellular reticular nucleus、lateral reticular nucleus與thalamic midline nucleus、habenular nucleus與paraventricular nucleus。
  綜合以上的結果,公孫穴與子宮之間的神經連結會透過延腦與間腦的神經核將訊息轉接,然後再藉由影響自主神經系統的功能,進而去調控子宮的功能。


英文摘要   Acupuncture is an ancient Chinese method to relieve pain and modify visceral functions in human and animals. Previous evidences indicated that the nervous system might involve in clinical effects about acupuncture. Although effects of acupunctural therapy have been examined in electrophysiology and neuropharmacology, the connections between acupoint and its target organ are not well established. Traditionally, Gong-Sun point has been wildly applied in patients suffered from gynecological diseases, such as dystocia and dysmenorrhea. The neuronal connections between Gong-Sun point and uterus pass through the spinal local circuit or the higher center is still controversial. Our study is aimed to make clear these blinding points.
  Firstly, the immunoreactivity of c-fos protein was examined following electroacupuncture (EA) on the Gong-Sun point. Secondly, the transneuronal tracer, pseudorabies virus (PrV), was injected into the uterus to identify the central pathway of uterus. Finally, the double labeling of c-fos and PrV were examined to establish to neuronal relationship between Gong-Sun point and uterus.
  The present results showed that the c-fos positive neurons were expressed in lamina I, II, IV, V, X at L2-L5 ; the PrV infected neurons were expressed in the T11-S2 dorsal horn and the L6-S2 and T11-L3 lateral horn. In medulla, the c-fos positive neurons were localized in gracile nucleus, gigantocellular reticular nucleus, parvocellular reticular nucleus and lateral reticular nucleus; and the PrV infected neurons were localized in nucleus tractus solitarius, dorsal motor nucleus of vagus, gigantocellular reticular nucleus, parvocellular reticular nucleus and lateral reticular nucleus. In diencephalons, the c-fos positive neurons were localized in thalamic midline nucleus, suprachiasmatic nucleus, paraventricular nucleus of hypothalamus, anterior perforating substance, habenular nucleus and amygdale. The PrV infected neurons were localized in thalamic midline nucleus, habenular nucleus and paraventricular nucleus. Finally, the c-fos and PrV double labeled neurons were observed in the gigantocellular reticular nucleus, parvocellular reticular nucleus, lateral reticular nucleus of medulla and the thalamic midline nucleus, habenular nucleus, paraventricular nucleus in diencephalon.
  In conclusion, these results suggest that the acupuncture of Gong-Sun point might affect uterine function via the complicated indirect pathway in medulla and diencephalon.
