中文摘要 背景:憂鬱症患者由於憂鬱情緒,降低身體活動動機,致減少活動參與。如何透過促進其自我效能提升身體活動,進而改善憂鬱情緒,是重要議題與挑戰。
目的:驗證以自我效能架構設計身體活動介入方案對改善社區憂鬱症病人之活動參與及憂鬱情緒改善之成效。
方法: 以單一受試研究法進行本研究,研究設計採三階段A-B-A跨參與者的多基線設計(Multiple baseline across subjects)。研究對象來自南部社區憂鬱症病人共六位。以自我效能架構設計身體活動方案,活動過程共三期(A1-B-A2),A1為基礎觀察期5天,B為介入期10天,A2維持期5天,每位共参與20天。其中介入期的身體活動種類依受試者喜好選擇,每次介入20鐘,結束後與他人分享活動的感受等。研究工具採用台灣人憂鬱量表 (Taiwanese Depression Questionnaire, 簡稱TDQ)測量憂鬱情緒,及國際身體活動量表(International Physical Activity Questionnaire, IPAQ)短版問卷測量活動情形。TDQ憂鬱量表於A1觀察期測量3次,B介入期測量4次,A2維持期測量3次共10次。IPAQ身體活動量表於A1觀察期前與A2維持期結束後進行測量共2次。採C統計及成對樣本T檢定進行資料分析,考驗相鄰階段間資料點的變化趨勢有否達顯著水準,及檢定身體活動效能方案介入前後受試者身體活動量之差異,以驗證身體活動效能方案是否可改善憂鬱情緒及促進憂鬱症病人的活動參與量。
結果:經由C統計驗證受試者經身體活動效能方案其憂鬱分數有下降趨勢,其中平時有規律身體活動者,其緩解憂鬱狀態較具持續效果。在身體活動量以成對T檢定,在費力活動、中等費力活動或走路超過十分鐘以上等活動量有明顯增加。
結論:身體活動效能方案的介入可明顯促進社區憂鬱症患者身體活動量,緩解其憂鬱情緒。未來社區專業人員可嘗試應用自我效能架構設計憂鬱症者的身體活動介入方案,尊重自主性、鼓勵選擇其喜好之活動方式,以促進其身體活動與緩解其憂鬱情緒。
英文摘要 Background: Depression patients due to depressed mood, reduce physical activity motivation, so as to reduce participation in activities, how to improve physical activity by improving their self-efficacy and improve their mood are important issues and challenges.
Purpose: The purpose of this study was to validate the design of self-efficacy elements of physical intervention program to improve community participation in the activities of depression, and to assess the effectiveness of depression improvement.
Method: This study was conducted in a single subject study, The study consisted of six patients with depression in the southern community. To design a physical activity program with a self-efficacy framework, the process is divided into three phases(A1-B-A2). A1 is based observation period of 5 days, B is for the intervention period of 10 days, A2 is maintenance period of 5 days. Each subject had to undergo a total of 20 days of intervention. The type of physical activity according to subjects preferences, each execution 20 minutes, after the end of sharing the feelings of other activities. The research tool is to use the Taiwanese Depression Questionnaire to measure depression, and used the International Physical Activity Questionnaire (IPAQ) short questionnaire to measure activity. The Taiwanese Depression Questionnaire was measured three times during the A1 observation period, B intervention period measured 4 times, A2 maintenance period measurement 3 times , a total of ten measurements were taken. The International Physical Activity questionnaire short-form was measured 2 times before the A1 observation period and after the A2 maintenance period, a total of two measurements were made. The C-statistic and the paired sample T-test were used to analyze the data. Examining whether the trends in data points between adjacent stages are significant, that to assess the effectiveness and differences of physical activity program before and after intervention in physical activity of the subjects, In order to test whether the physical activity program can improve depression and promote the activities of patients with depression participation.
Results: The C statistical verification found that subjects after physical activity program, depression scores have a downward trend. If in normal time to develop the regular pattern to do physical activity, relief of the state of depression is also more retain the effect. In the amount of physical activity is significantly improved, assumptions and validation are performed using the paired T-test. The results showed there was a significant increase in activities such as effortlessness, moderate effort or walking for more than ten minutes.
Conclusion: Physical activity program intervention can significantly promote community depression patients with physical activity, relieve their depressed mood. Future, community professionals can attempt to design a self-efficacy framework for designing physical intervention programs for people with depression. Respect for autonomy, to encourage the choice of activities of their preferences in order to promote their physical activity and ease their depressed mood.
