中文摘要 植牙過程中,牙醫師贗復缺牙患者常依據鄰近牙的齒列高度為製作假牙高度之判斷,但如患者為全口無牙時只能依賴牙技師傅的經驗及考量患者本身條件決定齒列高度及牙冠外形,盡量恢復患者咬合功能。本文旨在協助醫師針對全口無牙患者排牙時決定齒列高度及補人工骨的一套統計方法。
本研究共蒐集35位無缺牙、無矯正、無牙周疾病、或過去有重大齒列治療的受測者,均為台灣本土人士,經由迴歸分析來預測個人下顎特徵與下顎骨高度之空間關係,提供醫師補綴的參考。同樣以迴歸分析,找出下顎與莫森球面(sphere of Monson)之關係,得到莫森球半徑與下顎相關性,藉以推導出莫森球球心所在,針對全口無牙患者以存在的下顎骨特徵逆運算求取莫森球半徑及其球心,牙技師在排牙時可根據莫森球面排牙,並根據建議高度製做出最適患者咬合的全口假牙。最後本文更增加蒐集一般人以驗證公式的精確度,最後討論本文的方法應用於臨床實驗全口無牙的實例。
英文摘要 The heights of neighboring intact teeth are taken into account as one of essential references for dentists performing treatment. Yet in cases of edentulous patients, little information is present and thus dentists can only rely on their own experiences. As a result, satisfying outcomes are hardly achieved. The main concept of this study aims to assist dentists with a statistical method to treat prostheticing edentulous patients.
In this research, 35 native healthy Taiwanese volunteers are collected. All volunteers have intact complete teeth without previous or current orthodontic treatment, periodontal disease. Regression analysis is applied to obtain the relationships between dentition and heights of mandible teeth in space, and thus concludes a reference for bone filling before dental implant. With the analysis of mandibular feature points and height of alveolar bone, the relation within can be formulated. Furthermore, the mandibular feature points and sphere of Monson are closely inspected to reason out the relative function between the mandible and the radii of sphere of Monson. Once the center of sphere of Monson is determined, the center and occlusal radiu is well assessed. Technicians can set up the prosthesis height and dental crown according to the sphere of Monson. Finally, the accuracy of relative equation is verified by applying it in normal healthy patients. Hopefully, the resulting function can be applied to pre-clinical trial in edentulous patients with dental implant treatment.
